NYS requires physicals for students in grades Pre-K, K, 2, 4, 7, 10, and those new to Otselic Valley Central School (any grade)

New York State requires that students in grades PK, K, 2, 4, 7, 10 and new entrants to Otselic Valley Central School receive a physical examination. This physical requirement is in addition to the sports physicals described separately. 

Community Memorial Hospital will be available at our school on several occasions to perform physicals on the students in these selected grades.

The physicals are free and include a complete head to toe assessment.

If your child is in 6th or 7th grade, they may require a Tanner assessment if they indicate to us they are going to play a sport either this year or next year.

If your child has already had a physical examination by their own practitioner, please have the doctor’s office fax a copy to our school nurse, Mrs. Stacie Morse, RN, prior to May 1, 2017. Our fax number is 315-653-7500.

If we do not have proof of a physical in your child’s medical chart by May 1, 2017, a physical will be performed by Community Memorial Hospital at our school.
